Criteria to Qualify:

How to Earn Points:  (one point value awarded per assignment, not to be combined)

Point Value	Criteria
1	Complete one 13-week assignment
1	Placed Referral for 13 Weeks Points are in addition to Referral Bonus payout!
1.25	New for 2010! Complete one 13-week extension assignment
1.5	One 13 week assignment in association or bonus state
1.25	New for 2010! Complete one 13-week Home Health Assignment (REHAB ONLY)

Loyalty Bonus Points: (in addition to above!)

Point Value	Criteria
.5	All travelers who qualified for 2009 trip
1	All travelers who qualified for both the 2008 trip and 2009 trip or bonus

Don’t forget to send us referrals all year long for trip credits + CASH!

• Qualifying Dates: 12/1/2008 – 1/31/2010. Credit only given for assignments / placed referrals that begin during this time.
• Credit will be pro-rated for assignments shorter/longer than 13 weeks, however - No points given for assignments (or a referral’s assignment) that are voluntarily terminated or terminated for cause. CMG reserves the right to cancel/withdraw candidate's qualification at management discretion at anytime.
• Travelers must complete a minimum of 95% of total contracted hours for all assignments awarded points.
• Travel Teams must have shared housing arrangements for at least one 13 week assignment during the 2010 qualification period and at least one traveler must have earned 4 points.
• In order to receive Extension bonus extension assignment must start with no time off between previous assignment.
• The Guest’s ALL INCLUSIVE cost includes round trip domestic airfare (add’l chg may apply from AK & HI) from where guest lives plus 5 days land and transfer costs. Guest LAND, TRANSFER package does not include airfare.
• CMG will not be responsible for any additional fees directly charged by airline carrier.
• Each traveler qualifying for the trip is eligible to utilize one guestroom and may not purchase additional rooms.
• Full details, disclaimers and instructions will be supplied to each qualified traveler no later than 12/31/2009.
• Association* States (RNs): AZ, KS, MD, MO, NC, NM, OH, SC
  *Facility must be part of the State Hospital Assoc.
• Bonus States (Rehab): CT, IL, ME, MO, NM, WA, WV
